MultiCare Connected Care, LLC, Chooses RelayHealth's Fuse to Create a Single-Source Data Platform

MultiCare Connected Care, LLC (MCC), an Accountable Care Organization with a comprehensive Clinically Integrated Network (CIN), has selected RelayHealth’s FuseTM to create an electronic information exchange platform that will support care coordination and financial insight amongst network members.

This platform will connect various electronic medical record systems, combining clinical, financial and operational data, to achieve a single information source. This single view of the data will be valuable in managing the health of populations and achieving the financial goals of MCC.

FuseTM is a data foundation, acquiring and aggregating clinical, financial and operational insight across care settings to provide data-driven insights to make confident decisions along their journey to Value Based Care. The foundation enables organizations to address their need for scalable data acquisition across care settings and establish toolsets that offer structure and flexibility which will highlight potential roadblocks on the journey to value. At MCC, the overarching goal is to help patients get, and stay, healthy through highly coordinated, accessible care. With the roll-out of FuseTM, the organization will address the following:

  • Longitudinal record for care coordination among disparate EMRs
  • Policy creation for network structure
  • Outline both current and future business needs
  • Identify data required for population dashboards to meet those needs
